📢 KONSULTA GUIDELINES

The Cordova Primary Health Care Facility encourages residents to bring the following requirements when visiting for Konsulta:

1️⃣ Valid ID
Patients must bring a valid government-issued ID to verify residency in Cordova.
Acceptable IDs include: National ID, PhilHealth ID, Postal ID, Driver’s License, Senior Citizen ID, PWD ID, Voter’s ID or Certificate, and other government-issued IDs.

• For minors, bring a School ID with Cordova address or a parent/guardian’s valid ID.

2️⃣ The Patient Must Be Present
It is important that the patient themselves attends the health center, not just the guardian, as vital signs need to be taken and consultation with the doctor conducted.

• If the patient is bedridden or unable to travel, the guardian may coordinate with the Barangay Health Worker (BHW) in their area.
